ACUTE KIDNEY INJURY IN HOSPITALIZED PATIENTS WITH PORTO-SINUSOIDAL VASCULAR DISORDER AND ASCITES

Background:
By definition, HRS-AKI can only be diagnosed in patients who have cirrhosis and ascites, therefore excluding patients with other causes of portal hypertension such as PSVD from the picture. However, important parallels can be made regarding the hemodynamic changes of patients with PSVD and those with cirrhosis, particularly once ascites develops.
We therefore hypothesize that hemodynamic and inflammatory changes related to portal hypertensin occurring in PSVD can lead to a similar pathophysiology of “functional” AKI as in cirrhosis.
Study Design:
Multicenter, retrospective, observational propensity matched-cohort study.
Inclusion Criteria:
Cases with PSVD: Adult patients with PSVD and ascites hospitalized between 2010 and 2026 for the treatment of liver disease-related complications (ascites, gastrointestinal bleeding, encephalopathy, infections, etc,), with AKI.
Controls with cirrhosis: Adult patients with cirrhosis and ascites hospitalized during the same period (2010 and 2026) for the treatment of liver disease related complications (ascites, gastrointestinal bleeding, encephalopathy, infections, etc.), with AKI. Cirrhosis controls will be obtained from an existing international database.
